In the beginning of the story, the relationship serms strained as the protagonist and his wife were constantly arguing about how the other doesn't care or understand about him or her. There seems to be a misscommunication between them in which the husband wants to prolong his wife's life as long as possible by going out to buy her the best medicine or food, but the wife just wants to spend her last days with him longer. Their differences in dealing with the wife's health results in arguments and estrangement. Their relationship turns for the worse as it shifts from being a husband and wife to that of a caretaker and patient. The wife's incurable illness also seems to be affecting their personality. The husband becomes stressed for wanting to help his wife, but he is also preoccupied with his work to earn more money to help his wife. The wife feels lonely without her husband and believes that her husband doesn't care about her and is only nursing her because of the responsibility as a caretaker, not because he cares about her as a husband. It's only when the wife realizes that the illness is what's changing her, and not their feelings for each other, does she understand her husband's feelings. And only when the husband realizes that his wife doesn't have much time to live does he realize the importance of spending time with her. It seems they have both accepted death as it is and was able to appreciate their last moments together.

I was really dying to know what was written in the letter too! I like to think that it was something which basically amounted to "everything I never told you," like all the positive things she felt about him and their relationship which she never got to tell him because of the illness. Maybe it's a collection of all the hopes she had for things they would do together, but were never able to. It could even be just a diary of her everyday life and thoughts (considering she was in that bed for so long, she probably had tons of time). I feel like the letter could be another story in itself. Or maybe like one those Nicholas Sparks movies, haha.
